Bilingual Providers Role 1: Bilingual RN with Chronic Care Experience

We're partnering with a digital health company that supports patients at risk for, or living with, chronic kidney disease (CKD), and we're looking for Bilingual Spanish-English Registered Nurses (RNs) to join their team. This is a fully remote, full-time opportunity for nurses who are passionate about coaching, patient education, and supporting behavior change in chronic care.

What You'll Do:

● Connect with patients via phone, video, and email to support their health goals

● Use motivational coaching to guide behavior change and assess health needs

● Develop individualized care plans (CKD experience preferred)

● Coordinate care and collaborate with providers as needed

What We're Looking For:

● Licensure: RN license in good standing, multi-state compact license preferred

● Experience: 5+ years of nursing experience

● Language: Bilingual Spanish-English required

● Additional Training: Certified Diabetes Care and Education Specialist and/or training in health coaching or motivational interviewing strongly preferred

● Location: U.S.-based remote. Preference for candidates in MT/PT, though candidates in other U.S. time zones will be considered if they can work the required schedule

● Workspace: Quiet, secure location with reliable high-speed internet

Schedule: Full-time, Monday–Friday. Multiple schedule options are available based on team needs and candidate availability:

● Schedule Option 1: 8am–5pm PT two days per week and 11am–8pm PT three days per week

● Schedule Option 2: Standard daytime hours, with at least two 11am–8pm PT shifts per week

Details:

● Compensation: $86,000–$100,000 annually for candidates based in MT/PT; $78,000–$85,000 annually for candidates based in ET/CT. Benefits include Health, Dental, Vision, PTO, and 401K.

Role 2: Bilingual RN - Must be based in Puerto Rico

We're partnering with a digital health company focused on helping patients improve medication adherence, and we're looking for Bilingual Spanish-English Registered Nurses (RNs) to join their team. This is a fully remote, full-time opportunity for nurses with backgrounds in patient coaching, chronic condition support, medication adherence, and health education.

What You'll Do:

● Connect with patients by phone, text, and video to support enrollment and ongoing engagement

● Provide motivational coaching, medication education, and chronic disease support

● Conduct nurse-led medication reviews, guide proper administration, and address barriers to adherence

● Review patient submissions and escalate clinical concerns as needed

● Collaborate with providers, pharmacies, and care teams to resolve issues and improve outcomes

What We're Looking For:

● Licensure: Active RN license in good standing

● Experience: 5+ years in patient engagement, care coordination, or related clinical work

● Language: Bilingual Spanish-English required

● Location: Must reside in, and maintain residency in, Puerto Rico

● Workspace: Quiet, secure location with reliable high-speed internet

Schedule: Full-time; evening and weekend availability required. Available shifts include:

● Saturday–Wednesday: 12–8pm, 1–9pm, 2–10pm, or 3–11pm ET

● Sunday–Thursday: 12–8pm, 1–9pm, 2–10pm, or 3–11pm ET

Role 3: Bilingual Enrollment Coach

We're partnering with a digital health company dedicated to medication adherence, and we're looking for Bilingual Spanish-English coaches, medical assistants, community health workers, and allied health professionals to join their team. This is a fully remote, full-time opportunity for candidates with backgrounds in health coaching, public health, or community health work.

What You'll Do:

● Communicate with patients via phone, text, and email to guide them through enrollment

● Leverage listening skills and empathy to evaluate patient needs and match them with appropriate programs

● Handle inbound and high volume outbound communication (primarily outbound) and manage patient demographics in Salesforce

What We're Looking For:

● Training: Certification as a health coach, medical assistant, community health worker, pharmacy technician, or similar role

● Experience: 3+ years in health coaching, health education, patient engagement, or a related healthcare role

● Language: Bilingual Spanish-English required

● Location: U.S.-based remote

● Workspace: Quiet, secure location with reliable high-speed internet

Schedule: 12pm–8pm ET, including weekends

Role 4: Bilingual Care Advocate in NC or PA

We're partnering with a digital health company focused on caregiver support, and we're looking for a Bilingual Spanish-English Care Advocate to join their team. This is a remote, part-time opportunity for candidates based in Western North Carolina or Pennsylvania.

What You'll Do:

● Support family caregivers across various topics such as health, in-home care, senior living, financial and legal while encouraging caregiver self-care

● Build a trusted relationship with caregivers via phone, email, and live chat

What We're Looking For:

● Language: Bilingual Spanish-English required

● Education: Bachelor's degree preferred, or equivalent work experience

● Experience: At least five years of personal or professional experience supporting family caregivers; experience with Medicaid populations and community-based resource navigation highly valued

● Location: Must be based in Western North Carolina or Pennsylvania (U.S.-based)

● Additional: Background investigation and drug screening required, as permitted by law

Schedule: Approximately 25 hours per week, with flexible scheduling between 8am–6pm CST, Monday–Friday
